How it works

1. 1

   Your enrollment payment covers your first treatment on or around enrollment and leaves a starting credit in your Soft Life balance.

2. 2

   Your first monthly payment is charged one month after enrollment. Twelve consecutive monthly payments are added to your balance on the same date each month.

3. 3

   Each treatment draws the full tier cost from your balance. Your appointment can be no sooner than 15 weeks after the last treatment, with sufficient balance and a current account. Nothing is due at routine treatment visits.

4. 4

   Your next appointment is pre-scheduled at each visit for 15 to 17 weeks out. Soft Life 40 and 64 are funded for treatment at 15 weeks or later; Soft Life 100 typically funds treatment approximately every 4 months.

5. 5

   After the initial 12-month term, your plan continues month to month at the then-current rate until cancelled with 30 days written notice or cancelled by the practice.

The terms, plainly

Commitment

Enrollment payment plus 12 consecutive monthly payments from your enrollment date, then month to month at the then-current rate with 30 days written cancellation notice.

Units and balance

Tier units apply per treatment visit. Unused units do not roll over, accumulate or convert to credit or other services. Your monetary Soft Life balance is separate and may only fund Soft Life treatments.

Billing

A valid card on file is required. Monthly payments are due regardless of appointment timing. If a payment is declined, you have 7 days to update payment information; treatments and benefits are suspended until your account is current. You may request a balance statement at any time.

Remaining balance

At the end of the initial term, your balance carries forward if you continue month to month. If you do not continue, choose to apply it to one final treatment and pay any difference at that visit, or receive a refund to the card on file.

Scheduling

Treatment requires at least 15 weeks since your previous visit, a current account and enough balance to cover the full tier cost. Cancel or reschedule at least 24 hours ahead. Late cancellations incur a $50 fee payable before the next treatment. No-shows incur the $50 fee and forfeit that session without refund, rescheduling or credit. Monthly payments continue during appointment gaps.

Medical deferrals

If your provider defers treatment for a medical reason, the visit is rescheduled without penalty or fee. For an extended medical contraindication, the practice may pause the plan and billing, extending the term by the length of the pause.

Eligibility

Ages 18+. Xeomin is a prescription medical treatment; intake, medical history and neurotoxin consent are required. Your provider determines dosing and injection sites within the tier allowance.

Cancellation

Cancel within 3 business days of signing by written notice for a refund less any treatment at the regular $12 per unit rate. Other early cancellations reprice treatments received at $12 per unit, apply amounts paid and settle any amount due or refund remaining credit. Cancellation without penalty is allowed for death, permanent documented medical inability to receive treatment or relocation more than 25 miles from the practice.

Benefits and rate

Treatments and benefits are nontransferable and for the enrolled member only. Discounts cannot be combined with promotions, other discounts, memberships or packages. Your tier rate is locked for the initial 12 months. Month-to-month terms may change with 30 days notice. The practice may cancel for nonpayment, misuse of benefits or conduct that violates practice policies.
