How it works
- 1
Start with the $250 comprehensive evaluation. Initial women's hormone labs are $175.

If you are a candidate, choose the Core or Complete tier and enroll.
- 3
Follow-up labs are drawn every 8 weeks after starting or changing a dose, then quarterly, then yearly once stable.
- 4
Women's follow-up labs are $100 and may be drawn in office or at Labcorp.
The terms, plainly
- Eligibility
- Enrollment does not guarantee treatment. Clinical eligibility is determined by the treating provider after evaluation, labs and history review.
- Billing
- Recurring monthly billing to the card on file, renewing until cancelled. Declined payments must be updated within 5 days.
- Not included
- Follow-up labs, retail pharmacy costs on the Core tier, medications outside your hormone plan and advanced testing beyond the baseline panel.
- Cancellation
- The Balance Protocol is month to month with no long-term commitment. Full refund within 3 business days of signing. After that, cancel any time with 30 days written notice; the $50 enrollment deposit credits your final month.
- Insurance
- Cash-pay practice. Insurance is used only for your own pharmacy fills on the Core tier and, if elected, lab charges billed by the lab.
Clinical care is governed by your treatment plan, the BHRT Informed Consent and the patient packet on file. Full terms are in the Balance Protocol Membership Agreement (Form BHRT-M-01).
